MISCELLANEOUS SERVICE/ATTESTATION OF DOCUMENTS:
Please apply only to the consulate of the
appropriate Jurisdiction
All applicants are advised to ensure that the correct postage stamp is used. We regret that we cannot dispatch mails without correct stamps as it will not be accepted by
USPS.
To obtain the services listed below, applicant/s should fill in “the Miscellaneous Service Form”
- Birth Certificate/Registration of Birth
- No Objection Certificate to carry dead body/ashes
In the event of death of a relative/ friend, if the applicant
wishes to carry either body or ashes back home to India, the following
documents are required to be submitted along with the “Miscellaneous Service Form”
a. Death Certificate in original with one photo copy
b. Burial / Cremation or embalming certificate in original with one copy;
c. A Certificate from a Medical Officer stating that “ the deceased was not suffering from any contagious diseases” ;
d. Passport of the deceased [In case of US/Foreign national of Indian origin, only copy of US/Foreign Passport is required.]; and
e. Miscellaneous Service Form to be filled in requesting for the issue of a No Objection Certificate. The first three Certificates (a, b and c) will be attested, the Passport (d) cancelled and returned by the Embassy along with a No Objection Certificate. All these documents are to be carried by the person accompanying / carrying the body/ashes. Fee will depend upon the number of documents attested. The Miscellaneous Service Form should have the details of the deceased and signed by the person submitting the application along with copy of his passport.

- Attestation of foreign public documents- "Embassy
of India will not re-authenticate a public document that has been
apostilled by the authorities of the country of origin." As per Hague Convention, to which India and USA are signatories, abolishes the requirement of legalization for Foreign Public Documents. With the certification by the Hague Convention ‘ Apostille’, the document is entitled to recognition in India and no certification by the authentications office or legalization by Embassy of India or Consulates are required. Relevant provisions are contained at Article 1 of the ‘ Convention abolishing the requirement of legalization for foreign public documents’ which is reproduced below:-
Article 1
The present convention shall apply to public documents which have been executed in the territory of one Contracting State and which have to be produced in the territory of another Contracting State.
For the purposes of the present Convention, the following are deemed to be public documents:
a) documents emanating from an authority or an official connected with the courts or tribunals of the state, including those emanating from an public prosecutor, a clerk of a court or a process-server(‘huissier de justice’):
b) administrative documents :
c) notarial acts:
d) official certificates which are placed on documents signed by persons in their private capacity, such as official certificates recording the registration of a document or the fact that it was in existence on a certain date and official and notarial authentications of signatures.
However, the present Convention shall not apply:
a) to documents executed by diplomatic or consular agents:
b) to administrative documents dealing directly with commercial or customs operations.

- Attestation of documents for sale/ purchase of ship/ vessel

- Attestation of Power of Attorney (POA) (the applicant is required to affix one photograph at the end of the subject matter on the POA, specimen signature below the photograph and the passport details. This photograph along with other details on the POA must be notarized by a local Public Notary before submitting the application form in the Embassy. The Stamp/Seal of the Notary must cover some portion of photograph pasted on the POA.
(If the application is submitted in person by the applicant at the counter, notarisation is not necessary but the applicant has to sign the document at the counter.)
The applicant has to submit one additional identical photograph, original passport with its photocopy with the application form). In case, there are more than one signatories on a POA, the above requirement is to be completed by each individual. If no space is available on the original POA, a separate sheet may be attached as an enclosure to the original POA and should be notarized. The additional photographs must have their names printed on the reverse of the respective photograph.
Each document should also have signatures and address of two witnesses.
- Certificate of Child Abuse Clearance. In order to obtain "Certificate
of Child Abuse Clearance" applicants are required to fill up the miscellaneous
services form. The consular fee for such service is US $20.
Applicants who wish to obtain such certificate by mail must send an
additional US $20 as mailing fee. They should send two separate
certified checks/money orders of $20 each. Applicants who want to get the
certificate by mail should also send a notarized copy of their passport
where as applicants applying at the consular counter should bring their original
passport as well as a notarized copy of the passport. Two photographs of
the applicant should be attached with the miscellaneous form. The
processing time will be 5 to 7 working days.
