Embassy of India, Washington DC is authorized to issue subsequent extensions of Statement of Need Certificate to doctors based in its area of jurisdiction. For renewal of Statement of Need Certificate, the following documents may be submitted to the Embassy as part of the application:

A written assurance in original in the PRESCRIBED FORM, undertaking to return to India at the end of the training. 

A copy of the contract with the Hospital/Institute on the standard agreement form with a letter from the program director on the official letterhead with seal and contact telephone number. 

Copies of the initial Statement of Need Certificate issued by the Ministry of Health and  previous statement of need issued by Embassy/Consulate. 

Current DS-2019/IAP-66. 

Copy of Bio-Data with address, telephone number and email address. 

Copy of passport showing personal particulars and validity of visa and passport.
