MISCELLANEOUS SERVICE/ATTESTATION OF DOCUMENTS:
To get the following services, the applicant should fill in
“the Miscellaneous Service Form”
- Birth Certificate/Registration of Birth
- No Objection Certificate to carry dead body/ashes
In the event of death of a relative/ friend, if the applicant
wishes to carry either body or ashes back home to India, the following
documents are required to be submitted along with the “Miscellaneous Service Form”
a. Death Certificate in original with one photo copy
b. Burial / Cremation or embalming certificate in original with one copy;
c. A Certificate from a Medical Officer stating that “ the deceased was not suffering from any contagious diseases” ;
d. Passport of the deceased [In case of US/Foreign national of Indian origin, only copy of US/Foreign Passport is required.]; and
e. Miscellaneous Service Form to be filled in requesting for the issue of a No Objection Certificate.
The first three Certificates (a, b and c) will be attested, the Passport (d) cancelled and returned by the Embassy along with a No Objection Certificate. All these documents are to be carried by the person accompanying / carrying the body/ashes. Fee will depend upon the number of documents attested. The Miscellaneous Service Form should have the details of the deceased and signed by the person submitting the application along with copy of his passport.

- Attestation of affidavit /document for commercial purpose (Applicant must get the documents authenticated by the State Department of the State where the company/business is located and counter authenticated by the U.S. Department of State, where ever applicable)
- Attestation of documents for sale/ purchase of ship/ vessel
- Authentication of Indian Driving Licenses
- Attestation of Power of Attorney (POA) (the applicant is required to affix one photograph at the end of the subject matter on the POA, specimen signature below the photograph and the passport details. This photograph along with other details on the POA must be notarized by a local Public Notary before submitting the application form in the Embassy. The Stamp/Seal of the Notary must cover some portion of photograph pasted on the POA. The applicant has to submit one additional identical photograph, original passport with its photocopy with the application form). In case, there are more than one signatories on a POA, the above requirement is to be completed by each individual. If no space is available on the original POA, a separate sheet may be attached as an enclosure to the original POA and should be notarized. The additional photographs must have their names printed on the reverse of the respective photograph.
Note: For all the above mentioned services, the applicants has to
submit the original passport with its photocopy. |
